Thank you for taking the daily health screening survey. This page provides next steps if you answered "Yes" to any of the following questions in the survey.

A fever of 100.4 or above
You may not return to the workplace until:

• At least 3 days (72 hours) have passed since the fever has resolved (without the use of fever-reducing medications) AND
• At least 10 days have passed since the fever first appeared.
• Note: The above criteria do not apply to anyone who, after showing symptoms, receives a negative COVID-19 test.

Symptoms consistent with COVID-19 within the last 14 days
You may not return to the workplace until:

• At least 3 days (72 hours) have passed since symptoms have resolved (without the use of fever-reducing medications) AND
• At least 10 days have passed since symptoms first appeared.
• Note: The above criteria do not apply to anyone who, after showing symptoms, receives a negative COVID-19 test.

Been in close contact in the last 14 days with someone experiencing symptoms
You may not return to the workplace until:

• Either 14 days have passed since the last close contact with the sick or symptomatic individual OR
• The symptomatic individual receives a negative COVID-19 test.

Traveled by airplane in the last 14 days
You may not return to the workplace until:

• 14 days have passed since you returned home from travel or you receive a negative COVID-19 test.

Been advised by a healthcare provider to self-quarantine
Inform your supervisor when you were told to self-quarantine and for how long. You may not return to the workplace until you have completed your quarantine as advised.

Tested positive for COVID-19
Inform your supervisor (i) when you were swabbed for the test, (ii) how many days it has been since you first experienced symptoms, and (iii) how many days you have been symptom-free.

You may not return to the workplace until:

• At least 3 days (72 hours) have passed since symptoms have resolved (without the use of fever-reducing medications) AND
• At least 10 days have passed since you were swabbed for the positive COVID-19 test.
